We collect information that is necessary to provide you with medical consultations and issue certificates for your WorkCover or CTP claim. This includes:

  • Identity information: Name, date of birth, address, phone number, email address.
  • Claim information: Claim number, insurer name, employer name, claim type (WorkCover or CTP), police event number (CTP).
  • Health information: Injury description, medical history relevant to your claim, clinical assessment findings, diagnosis, work capacity assessment, treatment recommendations.
  • Consultation records: Clinical notes, Certificates of Capacity, Certificates of Fitness, referral letters, invoices.
  • Technical information: IP address, browser type, device information collected when you use our website or booking system.

3. How We Collect Information

We collect information directly from you when you book an appointment (via our online booking form, phone, SMS, or WhatsApp), during your telehealth video consultation, and through correspondence with you, your employer, your insurer, or other treating practitioners involved in your care.

4. How We Use Your Information

We use your information to:

  • Conduct your medical consultation and clinical assessment.
  • Issue Certificates of Capacity (WorkCover) or Certificates of Fitness (CTP).
  • Deliver certificates and clinical documentation to you, your employer, and your insurer as required by SIRA.
  • Submit invoices to your workers compensation or CTP insurer for payment.
  • Coordinate your care with other treating practitioners (e.g., physiotherapists, specialists) with your consent.
  • Comply with legal and regulatory obligations, including SIRA reporting requirements.
  • Improve our services and communicate with you about upcoming appointments.

5. Disclosure of Your Information

We may disclose your personal and health information to:

  • Your workers compensation or CTP insurer (for certificate delivery and billing).
  • Your employer (Certificate of Capacity as required under the Workers Compensation Act 1987).
  • Other treating practitioners involved in your care (with your consent).
  • Government agencies where required by law (e.g., SIRA, AHPRA).
  • Our technology service providers who assist in delivering our services (e.g., practice management software, video consultation platform), who are bound by confidentiality agreements.

We will not sell, rent, or otherwise disclose your personal information to third parties for marketing purposes.

6. Storage and Security

Your health information is stored securely in our practice management system, which is hosted in Australia and compliant with Australian healthcare data security standards. We use encryption, access controls, and audit logging to protect your information. Telehealth consultations are conducted via SIRA-compliant, encrypted video platforms.

7. Retention

We retain your health records for a minimum of 7 years from the date of last consultation, or until you turn 25 if you were a minor at the time of treatment, in accordance with the Health Records and Information Privacy Act 2002 (NSW) and professional obligations.

8. Your Rights

You have the right to:

  • Access the personal and health information we hold about you.
  • Request correction of inaccurate information.
  • Request information about how your data has been used or disclosed.
  • Make a complaint about how we have handled your information.
